This document contains my understanding and agreement with St. Olaf College and the St. Olaf College Archives with respect to my participation in the written, video-recorded, and/or audio-recorded reflection that I am submitting via this site. I hereby certify that I created, and own the copyright to, the submitted material and that I am at least 18 years old. I have reviewed my audio recording prior to submission.  I understand that the recording and resulting transcripts may be displayed and made publicly available by the college, and will be made part of the St. Olaf College Archives’ collections and will be available to future students, scholars, and researchers.  Additionally, I hereby grant to St. Olaf College and the St. Olaf College Archives, at no cost, the perpetual, nonexclusive, transferable, worldwide right to use, reproduce, transmit, exhibit, display, perform, broadcast, create derivative works from, distribute, and authorize the redistribution of the materials in any medium. By giving this permission, I understand that I retain any copyright and related rights that I may hold.

I hereby grant to the St. Olaf College Archives a license to add the material to its archival collections and preserve it for future generations of students, scholars, and researchers.  I give the archives, acting through its agents, employees, or representatives, permission to organize the materials according to accepted archival principles; to create metadata, finding aids, and full-text search interfaces required for the preservation and discovery of the materials; and to make the materials accessible to researchers and staff.  I understand that all materials (including physical property) resultant from my submission will become the property of St. Olaf College and the St. Olaf College Archives.  At the discretion of St. Olaf College and the St. Olaf College Archives, my materials, or any part thereof, may be disposed of by St. Olaf College and the St. Olaf College Archives at any time in accordance with its procedures for disposition of materials no longer needed or required for its collections.

I waive all claims for compensation and any right to inspect or approve, use, or re-use of the materials I submit. I hereby release St. Olaf College and the St. Olaf College Archives, and its assignees, successors, and designees, from any and all claims, damages, and demands arising out of or in connection with the use of the materials, including but not limited to any claims for copyright infringement, defamation, invasion of privacy, or right of publicity.

I understand and agree that the submitted material may contain sensitive personal information about me, including personal health information, and that this information may be archived and shared as described above. I agree not to include personally identifiable information of (or information that could allow a third party to identify) other individuals, including but not limited to students or employees of St. Olaf College — this includes their personal health information — unless those individuals have formally agreed to it by completing this form.
